Program Director - Integrated Health Home

In this role, you will oversee all program goals and measures.  The position will oversee the IHH team to ensure policies, procedures and standards are followed.  The Director maintains accountability for the implementation, ongoing management, evaluation, and continuous improvement. Compensation (based on experience) range: between $56,700 and $79,400 per year
Work Schedule: 40 hours per week

  • Bachelor’s degree in Social Work, Public Administration of other Human Service Degree required. Master’s Degree preferred.
  • Three years of experience in human service field
  • Five years program management and supervisory experience

 

WHAT YOU’LL DO-

  • Build and maintain a solid team of health care focused case coordinator professionals to integrate medical, social, and behavioral health care needs for individuals with serious mental illness and emotional disturbance.
  • Become literate in Patient Centered Medical Home (PCMH) principles, population management methods, team-based care, chronic care models, care coordination, peer support models of care.
  • Market the IHH program to the community, including physical health partners and clients.
  • Incorporate a data management program to manage diagnostic populations and improve health outcomes within the IHH.
  • Manage program budget with special emphasis on meeting program outcomes to acquire all possible payments including the 25% payment for positive outcome achievement.
  • Partner with funders, health coaches and all other identified contractor partners as it relates to program implementation.
