SUMMARY DESCRIPTION OF WORK

Professionals at this level have excellent communication skills with clients and community/hospital providers. Specific project, program or office responsibilities may occur at this level.  Decision-making, problem solving, and highly developed interpersonal skills are critical.  Experience in case management is preferred. The Behavioral Health Case Manager will assess clients who are in the hospital or emergency department at Jennie Edmundson and link them with behavioral health services in the community if needed as part of Caring For Our Communities of Southwest Iowa. 

Work Schedule: Monday through Friday, 9:00 a.m. to 1:00 p.m.

Essential Duties and Responsibilities

  • *Screen client for behavioral health needs.
  • *Attend daily huddle at Jennie Edmundson hospital for care coordination.
  • *Link clients to behavioral health services in the community.
  • *Provide support and consultation to team members and community partners
  • *Maintain all contact documentation and comply with contract conditions.
  • *Maintain referral database and complete required reports.
  • *Completes necessary referral and program paperwork.
  • *Coordinate services with other community agencies and resources.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.  
  • *Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • *Create, maintain and share as appropriate a dynamic self-care plan.
  • * Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • * Essential functions of this job are to be performed on company physical work site
  • Performs other program related duties as assigned.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in Human Service field preferred
  • Two years experience in Behavioral Health.
  • Valid driver’s license, acceptable driving record.
