SUMMARY DESCRIPTION OF WORK
This Case Manager will work with Samaritan Housing Program. The Samaritan Housing Program will provide scattered site Permanent Supportive Housing to chronically homeless individuals in Douglas, Sarpy, and Pottawattamie Counties in a low-demand setting. The Case Manager will work to identify, screen, and serve eligible clients through the program.
Intensive case management responsibilities begin at this level. Educational requirements are very specific. Decision making, problem solving, and highly developed interpersonal skills are critical. This position requires high level of social work skills, practice and knowledge
ESSENTIAL DUTIES AND RESPONSIBILITIES

Bachelor's Degree in Social Work, or related field and two years experience
Valid Drivers License and acceptable driving record.
