About this position:

As the Habilitative Specialist/Case Manager you will be empowering people served to gain independence. You will be developing person centered planning for XX individuals receiving Mosaic’s services, facilitating the care planning meetings and training those who serve on the desired outcomes for each individual.

Individualized person centered services are the focus for each person receiving services through this agency so monitoring each service plan, goal development and staff training to ensure individual outcomes are achieved is crucial for success This leader will need to be flexible, have excellent communication skills for internal and external partners and manage details while meeting deadlines.

* Bachelor's Degree in Psychology, Special Education or related field OR can substitute 5 years of experience in similar position/field of work for education.
* Minimum of one year of experience working with individuals with developmental disabilities required.
* May require QMRP certification and experience in an ICF-MR.
* Work requires frequent physical activity including extended periods of standing, walking, and bending with occasional.
* Work also requires constant reaching between knee and shoulder level as well as frequent reaching below knee level and overhead.
* Work requires occasional independent lifting up to 25 pounds, frequent push/pull up to 40 pounds of force and the ability to safely transfer 50 pounds.
