Overview

This position assists frail seniors to enjoy their highest level of independence while receiving the necessary care and service to meet their changing needs. Care Partners assist with restorative care and administration of medications. This position partners with other teams to assure individualized dining, environmental, social and healthcare experiences. Care Partners are required to be Certified Nurse Assistant/Aides and Certified Medication Aide in addition to their responsibilities. Supports and lives out Immanuel's Mission and CHRIST Promises.

Responsibilities

Key Areas	Key Responsibilities and Duties of the Job
Resident Care	Participates in assessments. Partners with family and team to best meet resident needs. Participates in the development and implementation of individualized care planning. Assists with personal care for an assigned group of residents. Seeks direction and reports resident change in condition. Performs clinical procedures within scope of policies, procedures and licensure requirements. Partners with dietary team to assured individualized meals and snacks. Partners with environmental team to assure clean and safe residence. Partners with recreational, pastoral and wellness teams to assure participation in resident selected activities.
Administrative & Compliance	Participates in all safety, security and infection control programs. Complies with all internal/external regulations, policies and procedures. Understanding and practice of quality standards and HIPAA regulations. Supports continuous improvement. Makes entries in the EMR consistent with and limited to the scope of job duties and applicable licensure. Assumes responsibility for personal and professional growth through participation in meetings, training and continuing education programs. Maintains professional affiliations and any required certifications.
Other	Performs other duties as assigned or requested.

Qualifications

Education-

Certified Nurse Aide/Assistant (CNA) certification
Certified Medication Aide (MA) includes completion of 40 hour minimum state approved course.
Active member in good standing on the applicable state registry for CNA and MA.

Experience-

Minimum of one (1) year experience as a Certified Nurse Aide.

Other Requirements

Certification in Basic Life Support and First Aid is preferred or willingness to obtain certification within 60 days of hire date required.
