**$2,000 Hiring Bonus**

 

The role of the Lead Care Partner - AL is to provide Christ-centered service to residents, their families, and co-workers.  Responsibilities in addition to performing duties of a Care Partner - AL may include training new employees in the role and responsibilities of their jobs, answering questions for Care Partners and Medication Aides, and assisting the Nurse Manager, Nurse Supervisor and/or Lead LPN in ways that are beneficial to the team and department. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

Resident Care

  • Offers resident assistance in personal care, health maintenance and daily activities of living such as bathing, grooming, dressing, transfers, ambulation, toileting and serving of meals and laundry.
  • Provides assistance to and from activities, appointments in accordance with service agreement while encouraging as much independence as appropriate for each individual.
  •  Administers medication as requested by the resident and in accordance with Iowa/Nebraska regulations governing the provision of medications by the Medication Aide-50/40 hour certification.

Documentation Duties

  • Performs accurate reporting and documentation in the electronic health care software using established procedures, policies and standards of Immanuel and State guidelines. Information may include, but is not limited to vital signs, weight, behavior and mobility. 
  • Documents resident incidents, accidents, emergencies or unusual behavior.  Completes administrative duties required or as requested such as copying and faxing.

Oversight

  • May oversee special projects or areas in accordance with state regulations for individual development.
  • Trains, coaches, and monitors progress of new Care Partners using orientation material appropriate to ensure competencies are met and answers question and/or assists them in performing a task not done previously. 
  • Ensures Care Partners and Medication Aides are completing their tasks in a timely and thorough manner in accordance with state regulations or as directed by the Lead LPN or Assisted Living Supervisor/ Manager.
  • Communicates any changes/information to the Lead LPN or Assisted Living Supervisor/Manager.

Compliance

  • Maintains a safe and clean environment.
  • Follows procedures for proper equipment use, infection control, disaster plans, per policy and regulatory guidelines.
  • Work is performed within the scope of his/her authority to practice in Iowa and/or Nebraska.

Other

  • Maintain registry requirements as determined by the medication act and Iowa/Nebraska Assisted Living Regulations including education hours, in-services, and/or reporting.
  • Performs other duties as assigned or requested.

QUALIFICATIONS

Education-

  • High school graduate or equivalent is required. 
  • Minimum 18 years of age according to state regulation and current Medication Aide 50 (Iowa) and/or Medication Aide 40 (Nebraska) certification is required.
  • Certified Nurse’s Aide is required.

Experience-

  • One (1) year of Care Partner experience at an Immanuel Community is required.
  • Work-related experience at another company is desired.

Other Requirements-

  • Certified as a First Responder in CPR/AED and First Aid by national standards or willingness to obtain within 60 days of hire.
  • Be legally authorized (certified) to practice the job’s functions and actions in Iowa – Medication Aide 50 Hour and/or Nebraska – Medication Aide 40 Hour.  Required to be actively listed on the Medication Aide Registry.
  • Be legally authorized (certified) with active Certified Nursing Assistant to practice the job’s functions and actions in Iowa and/or Nebraska.

 

KSA- Knowledge Skills and Abilities-

  • Knowledge of concepts, principles, and practices in a care giving function.
  • Knowledge of medical terminology, instruments and equipment and how to use and maintain them.
  • Knowledge of physical, mental, spiritual, and social needs of the senior and/or frail elderly population and their families.
  • Skilled in use of computer and certain necessary programs.
  • Skilled in transferring knowledge and methods to other employees.
  • Ability to develop relationships with residents, co-workers, and management.
  • Ability to effectively communicate verbally; make eye contact, pleasantly greet people, and use tact and diplomacy in interactions at all levels.
  • Ability to place a high value on treating other with dignity and respect.
  • Ability to work shifts as posted and/or required.
