Job Description

Purpose of the Job:

Manages the flow of clinic by leading all staff in providing medical and nursing care and ensuring high quality standards in collaboration with Clinic Manager.  Coordinates services with primary care team and acts as primary liaison with family and/or caregivers. Triages and assesses participants as needed. Acts as a gatekeeper for daily clinic scheduling.  Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, and communicates participant’s condition to caregivers, primary care and the interdisciplinary team. Teaches the participant, caregiver, family and staff in nursing care needed to meet their needs. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Areas

Key Responsibilities and Duties of the Job

35% Participant Care

  • Oversees and may conduct the comprehensive initial nursing assessment including problem identification and plan of care with new PACE enrollees.  Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
  • Oversees and may conduct an in-person nursing re-assessment semi-annually and as needed.
  • Implements the health care components of the established service plan.
  • Attends fall meetings and reviews/signs off on reports.
  • Monitors participant’s response to medical treatment plan and nursing interventions, and revises plan of care as necessary.
  • Reports and records results of the nursing assessment, care rendered, and the participant’s response to care.
  • Collaborates with other health care professionals and caregivers regarding provision of participant’s health care.
  • Oversees medication review of participants upon admittance  and discharge.
  • Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery.
  • Manages and reviews participant’s records, charts, pertinent information and staff’s documentation.  Documents as needed in medical records according to Immanuel Pathways standards.
  • Performs general skilled nursing care to participants.  Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. 
  • Provides support to Clinic RN’s with all processes as needed.
  • Assists in treatment, examination and testing of participants.
  • Assures certain health and personal care services are provided.
  • Educates participants, family and caregivers in regards to care, medications and treatments.

15% Staff Management

  • Provides orientation, initial and annual competencies, evaluates performance of respective staff members in collaboration with Clinic Manager.
  • Performs competencies on appropriate Participant Center staff as needed.
  • Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood.  Ensures that clinic staff works within their scope of practice.
  • Approves direct reports time cards.
  • Reports any staff issues to the Clinic Manager or Executive Director and works together on plan.
  • Engages in team building.  Coaches and mentors staff.  Instructs and teaches staff as needed.  Trains clinic staff to function independently.
  • Communicates with weekend and after hour staff, following up appropriately.
  • Educates other staff members to emergency procedures and Infection Control prevention and provides information to other staff and caregivers about health concerns and conditions of participants.
  • Educates staff in regards to care and treatment for the elderly as needed.

35% Clinic Operations

  • Manages flow of clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff.  Ensures timely and appropriate care of participants.
  • Responds to and perform Triage of incoming telephone calls.  Oversees scheduling of outside appointments for participants when necessary.
  • Actively oversees triage and medication management processes.
  • Oversees scheduling specialist doctor appointments and assists in hospital admissions and calls report to unit.
  • Takes lead in effectively communicating with interdisciplinary team community agencies at meetings and throughout the day.
  • Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel.  Ensures implementation of physician orders.
  • Coordinates participant care with outside contracted service providers – nursing homes, assisted living, lab, oxygen, etc.

10% Compliance

  • In conjunction with the Clinic Manager, implements the Infection Control Plan to ensure a safe and sanitary environment.
  • Follows all Immanuel Pathways policies and Procedures and OSHA safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  • Provides first aid treatments as needed.
  • Participates in and supports Quality Improvement initiatives to include medication errors and trends.
  • Acts within the scope of his or her authority to practice.

5% Other

  • Performs other duties as required or requested.
  • Supervisor for Employee Health Record system.
  • Participates in continuing education classes and any required staff and training meetings.
  • Maintains professional affiliations and any required certifications.



  • Professional nursing school graduate required.
  • Current License as a Registered Nurse (RN) in the respective state in which he/she is employed required.


  • Two (2) years of nurse management/charge nurse experience in a hospital, nursing home or community-based setting- preferably in a geriatric care setting. 
  • One (1) year experience working with the frail or elderly population is required.

Other Requirements-

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation
  • Ability to perform Basic Life Support (BLS) i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).
  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.

KSA- Knowledge Skills and Abilities-

  • Knowledge of P.A.C.E. regulations.
  • Knowledge of state healthcare and aging networks; in the respective state in which he/she is employed.
  • Knowledge of physical, mental, and social needs of the frail elderly and their families.
  • Knowledge of quality improvement and cost containment systems.
  • Knowledge of local health care and geriatric service networks.
  • Knowledge of PC computer systems, including word processing, spreadsheet, calendar and database.
  • Knowledge of the various disciplines to assess the needs of the elderly population.
  • Skilled in written and oral communication.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
  • Skilled in making decisions and leadership implementing principles.
  • Skilled in managing and supervising managerial and professional staff members.

  • Ability to effectively and efficiently plan, prioritize and follow-up on delegated responsibilities.

  • Ability to chart via Electronic Health Records.
  • Proven experience and basic computer proficiency (internet, email, Microsoft Office)
