Purpose of the Job:

Screens and assesses participant nutritional status for Immanuel Pathways.  Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met.  Performs nutritional assessment and provides nutritional education for Immanuel Pathways participants requiring interventions.  Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met.  Coordinates with contracted meal services to ensure meals meet the needs of the Immanuel Pathways participants.  Provides education to other health care professionals, as appropriate.  Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

50% Participant Nutrition

  • Conducts an initial comprehensive nutritional assessment on all Immanuel Pathways participants upon enrollment.  Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts and an in-person reassessment minimally on an annual basis or as needed.
  • Monitors monthly weights, medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes hydration status, intake, skin breakdown, reports changes to the Interdisciplinary Team and makes recommendations for changes to appropriate staff.
  • Documents nutrition notes and all other pertinent activities in the participant’s medical record according to Immanuel Pathways standards.
  • Provides nutritional counseling and education for Immanuel Pathways program participants and or caregivers using appropriate materials.
  • Responsible for the management of therapeutic diets, including modification to meet individual participant needs, and physician orders.
  • Assists in monitoring meals, including intake, diet compliance and tray accuracy.
  • Develops and implements the nutritional components of participant plan of care including active participation and development of therapeutic dining programs.
  • Maintains supplement/snack/bag lunch lists and dietary census for Immanuel Pathways participants.
  • Acts as liaison with contracted meal service to ensure meals meets the needs of the Immanuel Pathways participants.

35% Team Support & Development

  • Provides nutritional educational programs to Immanuel Pathways staff.
  • Functions as a member of the Interdisciplinary Team.  Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Performs initial and annual competencies on, and evaluates the performance of the Dietary Manager, Sous Chef, and Dietary Aide(s) if requested.
  • Ensures that the job responsibilities, authorities, and accountability of direct reports are defined and understood.
  • Oversees maintenance/cleanliness and orderliness of serving kitchen. Monitors portion control; appropriate serving of food; adherence to therapeutic diets, purchasing and inventory of food.

10% Compliance

  • Acts within the scope of his or her authority to practice.
  • Implements Exposure Control Plan.
  • Complies with Emergency Preparedness Plan.
  • Uses proper lifting and push/pull techniques, use gate belt assistance and participant transfers.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families. 
  • Assists in development of policies and procedures for nutrition and dining services.

5% Other

  • Performs other duties as assigned or requested.

 

QUALIFICATIONS

Education-

  • Bachelor’s or advanced degree from an accredited college with major studies in food and nutrition or dietetics is required.

Experience-

  • One (1) year of experience working with a frail or elderly population required.
  • Two (2) years of experience in the dietetics and/or nutrition.
  • One (1) year of management experience, preferably in a geriatric care setting required.

    Other Requirements-

  • Be legally authorized (currently licensed or applicable, certified or registered) to practice the job’s functions and actions in the respective state in which he/she is employed.
  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.
  • Agree to abide by the philosophy, practices, and protocols of the PACE organization.
  • Job-specific competencies for the Dietitian will be met prior to assuming participant care.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Incumbent may be asked to cover for a counterpart at a different PACE Pathways site to assess potential or current participant only.

KSA- Knowledge Skills and Abilities-

  • Knowledge of physical, mental and social needs of frail older adults.
  • Skilled in assessment of nutritional needs of frail older adults.
  • Skilled in written and oral communication.
  • Ability to chart via Electronic Health Records
  • Proven experience and basic computer proficiency (internet, email, Microsoft Office)
  • Ability to work with an Interdisciplinary Team, as well as independently.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, family members, and vendors.
  • Ability and willingness to provide assistance in a timely and professional manner.
  • Ability to use a continuous improvement approach in daily business practices.
  • Ability to exhibit high standard of ethics, accountability and responsibility.
  • Ability to be pro-active by seeking out opportunities to help rather than waiting to be asked.
