RN MDS Assessment Specialist - Full time days (1.0 FTE) - The Lighthouse

Job Description


The overall purpose of this job is to coordinate the completion of the Resident Assessment Instrument (RAI) process according to CMS time standards.  The primary responsibilities of this job include reviewing accuracy of assessment and MDS information to ensure correct reimbursement. Supports and lives out Immanuel’s Mission and CHRIST Promises.


Key Areas

Key Responsibilities and Duties of the Job


  • Schedule MDS per CMS Regulations
  • Open assessments
  • Manage sections of assessment being completed by others.
  • Sign and lock assessment prior to due date.
  • Submit to Federal database. 


  • Monitor need for COT’s and EOT’s.
  • Write comprehensive care plans.
  • Attend weekly and monthly meetings with staff members involved in the process.
  • Input data into Medicare log.
  • Attend discharge planning meeting.
  • Resource person for team leads. 


  • Performs other duties as assigned or requested.



  • Current Registered Nurse (R.N.) license in the respective state in which he/she is employed is required


  • One (1) year of experience in an assisted living or skilled care community or related experience is required.
  • One (1) year of scheduling or managing staffing levels in an health care environment.
  • Experience as san side (C.N.A. or Q.M.A) or R.N. is preferred.
  • Equivalent years of education may substitute for experience requirement. 

Other Requirements-

  • Certification in Basic Life Support and First Aid is preferred or willingness to obtain certification within 60 days of hire date.  
  • Certification in Resident Assessment Coordinator from accredited institution. 

KSA- Knowledge Skills and Abilities-

  • Knowledge of concepts, principles, and practices in the nursing field.
  • Knowledge of MDS assessment and assessment reference dates (ARDs).
  • Knowledge of RAI process.
  • Knowledge of Medicare and Medicaid guidelines
  • Knowledge of internet capabilities to find information, use searches, and open standard browsers.
  • Knowledge of physical, mental, spiritual, and social needs of the senior and/or frail elderly population and their families.
  • Knowledge of Immanuel, its mission and Promises, and how those are directly correlated to the incumbent’s job.
  • Skilled in effective and appropriate written communication.
  • Skilled in establishing and maintaining effective working relationships with residents/participants, co-workers, family members, and vendors.
  • Skilled in the use of computers including programs specific to the job
  • Ability to organize and effectively use time, resources, and talents.
  • Ability to exhibit high standard of ethics, accountability and responsibility.
  • Ability to plan effectively using proactive approach, keeping appointments and following through on commitments.
