Immanuel owns and operates ten independent and assisted living, memory support and long-term care retirement communities on six campuses in Omaha, Papillion and Lincoln, Nebraska and the Immanuel Pathways PACE Centers in Council Bluffs and Des Moines in Iowa and Omaha Nebraska.
POSITION PURPOSE: The Carepartner position is a unique position designed to assist frail seniors to enjoy their highest level of independence while receiving the necessary care and service to meet their changing needs. Carepartners assist with personal care including restorative care and the administration of medications. In addition this position partners with other departments to assure individualized dining, housekeeping and social and healthcare experiences. Carepartners are required to be Certified Medication Aides in addition to their other responsibilities.
The Certified Medication Aide is a position that has been created by the Nebraska legislature. The purpose of the position is to provide a safe way for individuals other than licensed health care professionals (doctors, nurses, etc.) to provide medications to individuals who are not able to take medications by themselves. The activities of a Certified Medication Aide are controlled by the Medication Aide Act. The purpose of the law is to “ensure the health, safety, and welfare of the public by providing for the accurate, cost-effective, efficient, and safe utilization of Medication Aides to assist in the administering of medications.
The Lighthouse is the long term and skilled care community located on the Lakeside Village Senior Living campus .The Lighthouse has 32 apartments and can provide residence for 36 residents.
NATURE AND SCOPE:
The incumbent reports directly to the Chief Nurse Executive. This position partners primarily with the resident to provide individualized resident-directed care and service. This position partners to provide housekeeping to meet the resident’s individual needs, assure individualized dining experiences, and assist with personal care including restorative care, comfort care and medication administration. The Carepartner plans and provides spontaneous and scheduled fun activities and identifies, responds to and consults with professional staff regarding changes in health conditions. The Certified Medication Aide works collaboratively with residents, families, physicians and all members of the Senior Services health care team. The Certified Medication Aide must be registered on the Medication Aide Registry. To be a Certified Medication Aide in any setting, the individual must pass a competency test. A competency test includes written questions and/or the demonstration of skills needed to provide medications. The testing will be done by different individuals in various work settings. To work in a nursing facility, the Certified Medication Aide must complete 40 hours of training. If the trainee is required to take a course in order to be a Certified Medication Aide in a particular work setting, the initial test will be given by the Department.
1. Participates in admission assessments and ongoing assessments to determine the resident’s needs and support to be provided. Partners with family and other team members to best meet resident needs and desires.
2. Participates in the development and implementation of individualized care planning.
3. Assists with personal care to an assigned group of residents. Partners with nurses and other Carepartners to assure that needs of all household residents are accommodated.
4. Seeks direction and guidance from licensed nurses (RN, LPN) regarding any change in resident condition or questions regarding any aspect of assigned work. Performs clinical procedures which do not require the skill/knowledge or judgment of a licensed professional and which may be safely delegated in accordance with Agency policies, procedures and state professional licensure requirements. Records and reports any changes in resident condition to supervising RN or charge nurse.
5. Administers Medications and treatments approved by Medication Aide Act. Documents all medications and treatments according to approved procedures Performs service procedures and treatments within scope of practice for state and per community licensure and policy. Includes the following Nebraska guidelines for medication Aides in daily practice:
Understands and practices confidentiality of information about the resident.
Recognizes the resident’s right to refuse medication.
Follows current standards of hygiene when handling medications.
Follows safe handling, storage, and disposal of medications.
Recognizes and reports unsafe conditions for medication provision.
Accurately documents medication provision.
Provides medications according to the Five Rights.
Provides medications according to individual resident needs and abilities.
Recognizes and reports general reactions to medications.
Safely provides medications for all ages by the oral, topical, inhalation, and instillation
Recognizes the limitations of the Medication Aide role.
6. Partners with dietary department to assure individualized meals and snacks.
7. Partners with housekeeping department to assure that each residence is maintained in a clean and safe manner.
8. Partners with activity, pastoral and social service teams to assure participation in meaningful activities of resident’s choosing.
9. Recognizes and knows how to report suspected adult abuse and neglect, and suspected child abuse. Recognizes the recipient’s property rights and physical boundaries.
10. Participates in all safety, security and infection control programs that are mandatory as well as those required and provided by the department.
11.Values and embraces multicultural traditions and recognizes when there is a need for and procures translations/interpretations appropriately.
12.Complies with all internal and external regulations, polices and procedures. Demonstrates understanding and practice applicable state and federal regulations, quality standards and HIPAA regulations. Supports facility efforts for continuous quality improvement.
13.Participates in defining Senior Services strategic priorities and in achieving goals and thus contributes to planning for use of physical, human and capital budget fiscal resources. Provides ideas, comments and recommendations through established forums, suggestion box, team meetings, steering groups, leadership communications and in any opportunity for open dialogue.
14. Demonstrates the ability to perform in an accurate and precise manner in crisis and emergency conditions.
15. Demonstrates the cost-effective awareness of careful, efficient, and nonwasteful use of supplies and linens. Uses established charge procedures; utilizes systems which support proper charging and cost reduction.
16. Assumes responsibility for personal and professional growth through participation in department meetings, inservice programs, and continuing education programs. Fulfills yearly education requirements as stated by policy.
17. Welcomes other duties as assigned.
18. Authorized to make entries in the medical record, consistent with and limited to the scope of the job duties and any applicable licensure.
19. Must have ability to work effectively and collaboratively in a diverse and multi-cultural environment.

One to three years experience as a CNA
Designation as a Certified Nurse Aide (CNA) Successful completion of a State of Nebraska approved medication aide course.
Listed on the Nebraska Health and Human Services Registry as a Certified Medication Aide (40 hour course) and Nursing Assistant
To be a Certified Medication Aide, the individual must be at least 18 years old and be of good moral character. The registration process includes completing an application form and returning it and registration fee to the Nebraska Department of Health and Human Services (Department). Information on the application form includes applicants full name, identifying information such as social security number, age, setting in which the applicant will provide medications, any convictions or pardons of a felony or misdemeanor, and a record of any training and/or competency testing required.
Every three years, competency must be retested and the registration on the Certified Medication Aide Registry renewed. A renewal notice will be sent to each Certified Medication Aide by the Department. It is the Medication Aide’s responsibility to keep the Department up-to-date ion the current address. Failure to do so may cause the registration to lapse. To renew a registry, send the completed renewal form, a record of recent competency test, and the required renewal fee to the Department. The activities of the Certified Medication Aide are also defined by the Act. A Certified Medication Aide can provide routine medications by the following basic routes:
Genuine interest in geriatric care. Compassion for seniors and those in a transitional stage of life. Ability to collaborate and work as a team member and leader
Ability to prepare and administer medication and treatments; using the 5 Rights – Right patient, right medication or treatment, right time, right dose, right route.
Good oral and written communication skills. Ability to organize and prioritize work, individual and group interaction skills. Job requires continuous reading skills, good verbal and written communication skills.
The caregiver must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residents served on his or her assigned unit. The individual must demonstrate the knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the resident’s status and interpret the appropriate information needed to identify each resident’s requirements relative to his or her age-specific needs as described in the department’s policies, procedures, or performance appraisal standards.
