Job Description

Purpose of the Job:

Responsible for providing and assisting participants of Immanuel Pathways program with activities of daily living, restorative and supportive care under the supervision of the Participant Center Manager, working with indirect supervision from the ClinicManager.   Assists with the organization and implementation of the activity programs. Monitors health status and provides a safe environment for participants.  May function as an Interdisciplinary Team member. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Requirements of Job:

  • Be legally authorized (currently licensed or if applicable, certified or registered) to practice the job’s functions and actions in the respective state in which he/she is employed.
  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.
  • Agree to abide by the philosophy, practices, and protocols of the PACE organization
  • Job specific competencies for the Participant Center Aide will be met prior to assuming participant care.
  • Incumbent may be asked to cover for a counterpart at a different PACE Pathways site on a fill-in basis for a short duration.
  • Incumbent may be assigned at more than one PACE Pathways site to perform job duties.

Key (Essential) Functions of this Job

70% Participant Assistance

Assists participants with daily activities, including yet not limited to:

  • Bathing, dressing, grooming, hair care, mouth care, hand and foot nail care and toileting
  • Eating meals at the feeding table
  • Maintaining clean and orderly day rooms, kitchen and personal care areas
  • Laundry including clothes for participants
  • Noting changes in participant’s physical or emotional health and communicating information to appropriate staff person
  • Accompanying participant to medical and other appointments, as well as helping the Driver load and unload participant from vehicles
  • Arranging participant’s take home supplies with Transportation Department
  • Maintaining participant’s medical records and fulfilling Pathways charting and reporting requirements

  • Demonstrating dependability through consistent compliance with scheduled work hours

  • Maintaining the confidentiality of all company procedures, results and information about participants, clients or families

5% Safety Standards

Follows safety policies and guidelines through:

  • Complying with OSHA (Occupational Safety and Health Administration) guidelines
  • Implementing Exposure Control Plan
  • Complying with Emergency Preparedness Plan
  • Using proper lifting and push/pull techniques, use gate belt assistance and participant transfers

10% Team Engagement

Engages in team planning and activities by:

  • Creating and participating in group and one-on-one activities in accordance with daily activity calendar and/or requested by Recreational Coordinator
  • Participating in the activities of the Interdisciplinary Team and carries out the plan of care as directed
  • Participating in and supporting Quality Improvement Initiatives

5% Other

Perform other duties as required or requested



  • High school diploma or equivalent is required.
  • Certified Nursing Assistant in the respective state in which he/she is employed is required.


  • One (1) year of experience working with the frail or elderly population required.

Other Requirements-

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

KSA- Knowledge Skills and Abilities-

  • Basic knowledge in working with frail and elderly to support Activities of Daily Living (ADL).
  • Skilled in identifying and recommending problem resolution.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.
  • Ability to work effectively with culturally, economically, and educationally, diverse populations.
