Wound Clinic RN Case Manager

40.00 hours / week Monday - Friday

• Graduate from an accredited school of nursing.
• Prefer Graduate of a WOCN accredited ET Nursing Education Program.
• Valid RN licensure required.
• 2 years of nursing experience in related specialty area.
• 2 years of Medical / Surgical nursing preferred.
• Must work independently with excellent organizational skills.
• Computer experience helpful.

• Provides appropriate care to ensure understanding and comfort level of treatment.
• Initiates decision-making and problem solving related to case management outcomes of patients with selected disorders of the GI, GU and integumentary status.
• Monitors and coordinates care of wound, ostomy, and continence patients delivered by all members of the health care team to achieve satisfactory patient outcomes.
• Maintains operational and clinical records to comply with professional standards of practice, federal and state regulations and established policies and procedures.
• Participates in wound, ostomy and continence care performance improvement program.
• Provides quality cost effective care to ensure financial stability.
• Assists with staff orientation/development to ensure competency of staff in the area of wound, ostomy and continence care.
• Initiates action in response to information indicating risk to correct, prevent, and/or minimize risk to employee, hospital, and patient.
