Position Description
Job Responsibilites
Provides patient focused treatments specific to the age of the patient to achieve maximum functional improvement by evaluating the patient’s rehabilitation and functional needs, setting goals, and implementing treatment plan components established through these findings by providing care as outlined in "Age Specific Criteria."
Accurately and efficiently performs job duties to meet patient physical therapy needs by delivering, and when appropriate, delegating to other health care team members, therapy services to the patient based on the patient assessed needs, physician orders, and patient preferences
Contributes to the interdisciplinary care plan to ensure quality patient care by documenting evaluations, setting and revising goals, evaluating and documenting progress, and participating in patient conferences.
Supports the patient care team and contributes to the delivery of patient focused care, through the exchange of information and coordination of treatment plan components, participation in the department orientation program of new employees, and by developing and maintaining cooperative working relationships with physicians and other health care team members.
Initiates action in response to information indicating risk to correct, prevent and/or minimize risk to the employee, agency, hospital and customers through knowledge of regulatory policies and procedures.
Maintains expected department productivity standards to ensure efficient operations by demonstrating effective time management skills, adaptability and flexibility in performing and prioritizing workload. This includes fluctuations in scheduled hours and on-call rotation to meet patient/department needs.
Job Qualifications
Graduate of an accredited school of Physical Therapy. Requires professional licensure in Nebraska
One year experience in health care setting
Computer experience helpful.
Must be able to work independently with excellent organizational and problem solving skills.
Requires good verbal and written communication skills.
