JOB SUMMARY

This position is responsible for providing Social Work services to Hospice clients and their families, as assigned.    Works with the volunteers to meet the needs of clients and families. 

This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.

KNOWLEDGE, SKILLS AND ABILITIES

This position requires effective counseling skills.  Should be emotionally mature, objective and sensitive, and should possess a basic concern for people and their problems, especially relating to aged, disabled persons.  Must be able to handle responsibility, work independently, and maintain good working relationships with clients and team members.  Requires basic knowledge of health care funding sources, including Medicare, Medicaid and insurance.  Must be able to speak, read, write, and understand the English language.

LICENSURE/CERTIFICATION

Must be certified by the State of Nebraska as a bachelor's level social worker.

Valid Nebraska driver's license required.

EDUCATION/TRAINING, AND WORK EXPERIENCE

Graduate of an accredited school of social work

One year of clinical experience appropriate to the counseling and case work needs of the terminally and/or medically ill required.

Home Health Care, Nursing home or hospice experience preferred.

Training and experience in dealing with grief preferred.
